Implements handoff item 38: tool execution preserves structured failure status.

  • Adds a typed result envelope with status, exit code, content, and artifact fields.
  • Keeps legacy string projections while allowing callers and guardrails to distinguish failures.
  • Adds regression coverage for shell, git, MCP, denial, and unknown-tool failures.
  • VERSION and package manifests bumped to 0.5.1.

Validation: Jerboa balance checks and git diff --check passed. The MCP verifier encountered its existing variable compile is not bound generated-library runtime error.
