Summary

Treat local open-file-input-port permission failures as per-file skipped uploads instead of aborting the bounded worker pool.
Preserve fatal behavior for transport, stable-file, and other unclassified upload failures.
Add regression coverage and advance the synchronized version to 2.0.19.
